RTG diagnostics in IDC Ružević

Before performing any dental procedure it is necessary to do an analysis of the condition of the patient’s oral cavity. The diagnostics is done in such a way that we do an examination of the patient’s oral cavity: first, with intraoral camera, and second, we do an X ray with the most modern digital RVG system with ten times less radiation than the usual RVG image.

Depending on the situation and the requested analysis we do:

  • Small intraoral RVG images – their advantage is that the image is displayed on the screen within seconds by digital sensor. The image is perfectly clear and, thanks to our system of image storing, it is always available.
  • 2D panoramic images (Ortopantomogram) – X ray image of both jaws. The advantages: the possibility of doing the X ray without the patient having to open their mouth, the area of large anatomical region, lesser amount of radiation, speed and the simplicity in using it.
  • 3D CBCT layered images of teeth and jaw. The advantage of this kind of image, CT COMPUTER TOMOGRAPHY, as opposed to a regular ortopantomogram, is in the layers of the image itself. The layers may go up to 0.5 mm and in that way, there is no possibility that we miss something in the analysis.

For the needs of surgery and implantology, and for analyzing the actual condition when planning the next implant or any other procedure, we always do a CBCT image with different layers which we use as a starting point for making a 3D image of the jaw.

In order to discover the initial dental plaque, invisible to the eye, we use the revolutionary method of laser plaque detection. With the help of the advanced DiagnoDENT we can detect even the smallest defects in the tooth structure, fast and efficiently, based on the difference in fluorescence between a healthy tooth and dental tissue infected with plaque. We guarantee top quality service to our patients with images that are permanently stored in our system and which can always be consulted.

Functional diagnostics and functional therapy in Implant Centre Ružević

Millions of people in Europe suffer from chronic headaches and take a large amount of analgesics. It is not a well-known fact, despite being very important, that it is the use of medicaments the very root of chronic headaches which ultimately leads to kidney disease. It is an even more obscure fact that 50-70 % of all headaches are linked to the teeth, masticatory and neck muscles and jaw joints. The formal expression for those functional diseases is mioartropathy, or in a larger context, CMD – craniomandibular dysfunction. A scientifically proven method which occupies with this problematics is functional diagnostics and functional therapy.

Functional disturbances of stomatognathic system

  • Bad prosthetic job, non-physiological configuration of masticatory surfaces, too high or too low constructions, non-physiological vertical or horizontal relation can lead to changes in the position of the head, cervical spine, back spine which can disrupt the entire statics.
  • The consequences are neuromuscular incoordination, which cause pain in masticatory, neck and shoulder muscles, but also causes headaches, neuralgic pain, migraines with similar symptoms, tinnitus, vertigoes (so-called descending pain).
  • Moreover, functional disturbances in the pelvis area, functionally short leg because of the compensatory spinal position, can lead to the change in the lower jaw position and, consequently, to the discoordination of the masticatory muscles with painful accompanying symptoms. A single systematic interdisciplinary work enables us to make the right diagnosis and conduct specific therapy.
  • Functional diagnostics is based on a clinical manual and instrumental analysis of the masticatory system (including masticatory muscles, jaw joint and the entire statics).
  • Functional therapy is based on the relaxation of the entire neuromuscular system (physio-manual therapy and special splint for the relaxation of the masticatory muscles). If needed, the correction of the position of the lower jaw is done.

Clinically, CMD manifests through various symptoms, such as pain in the masticatory muscles, jaw joint, face and head area, but also neck and shoulder muscles and down the spine. A chronic mental tension, stress caused at work or at home lead to unconscious undoing of the accumulated aggression through the teeth – bruxism. A badly shaped filling, crown or bridge lead to the increased tonus of the masticatory muscles and cause pain in that area, jaw joint, neck and shoulder muscles and in the spinal area. Naturally, the chronification of these kinds of pain reduces the life quality. Women are six times more likely to suffer from CMD than men are.

Cause and analysis of the disfunction

  • Stress
  • Psychological factors
  • Orthopedic factors
  • Occlusive disease
  • Psychological aspects (fear)
  • Physical factors (humidity, coldness)
  • Trauma (even a violent extraction)
  • Hormonal factors

It is of the utmost importance the fact that doctors and dentists are in possess of such a general knowledge of that area so that they can offer professional help to the ones who suffer of such a disease. Only one physiological relation of upper and lower jaw, just like the natural configuration of the masticatory surfaces with fillings, crowns, bridges and dentures, enable one physiological function of the masticatory muscles and jaw joints. Nowadays there are safe diagnostic methods on the basis of which it is possible to conduct efficient therapy. Besides the clinical and manual analysis, today the modern electronics is ever more often used in diagnostics since it shows even the smallest disturbances in the area of masticatory surfaces and jaw joints. Millions of teeth are being extracted every year without the real necessity for doing so since the pain that is felt is not caused by certain problem with the teeth, but instead the root of the problem is in an area of face, neck or shoulder muscles which later projects the pain in the teeth area. Basically, here we deal with the hyperactive node places in the head and neck muscles (trigger point) from where the pain is being projected to certain tooth. All it takes is the knowledge of the topography of trigger points, locate the node and eliminate it with a massage. So-called toothache will disappear instantly.

ARCUSdigma – support to functional diagnostics in treatment and therapy

Electronic analysis of movement in prosthetic therapy

The time spent for correcting the malocclusions and the danger of damaging the ceramics while performing the corrections can be totally or partially reduced with the help of analysis of movement. In a matter of few minutes, ARCUSdigma defines all the settings of the articulator and helps the dentist to determine the central position of the lower jaw.

Electronic analysis of movement in diagnostics

More and more patients are being diagnosed with the temporomandibular joint disorder. ARCUSdigma analyzes and documents the movements of the temporomandibular joint and the therapeutic process for the in-depth diagnostics. ARCUSdigma localizes painful temporomandibular positions using special, optionally available analytical software. The integrated calculator modulus enables office-oriented therapeutic realization of the finding, with KaVo PROTARdigma articulator, right after giving the diagnosis. Therefore, it becomes possible to realize, not only the occlusion necessary for the therapy, but also to perform for the first time the dynamical occlusion in a dental laboratory. So, the dentist is no longer limited to perform this time-consuming procedure in a dental chair.

Functional dentures Saving money, because of the massive reduction in adjusting the occlusion in the mouth of a patient.

  • Calculation of all the settings for KaVo PROTARdigma 9 articulator in less than ten minutes. PROTARdigma 9 articulator ensures all the conditions needed for designing a functional masticatory surface.
  • Mounting of the upper jaw model according to KaVo system of transition, or individual axis determination.
  • Analysis of central position of the lower jaw, for the mounting of the lower jaw model (optionally with electromyographic support).

This short demonstration indicates the importance of knowing the reliance and connection between functionally linked head, neck and spine structures in order to enable efficient therapy, with the right diagnostics (often interdisciplinary), which would set the patients free from chronic diseases that affect their work capacity and mental health. It all begins with jaw (temporomandibular) joints. Ideally, they are centred and symmetrical. The position of your jaw joints depends mostly on how normal your bite is. Depending on your bite, the joints are under the influence of the forces which can overload your jaw joints and, consequently, move them left, right, forward or backward. Therefore, each joint can be differently moved. Those different movements can result in a joint dislocation, that is, the joint disc displacement.

Multiple factors affect the possible disturbances (dysfunction) of the normal jaw joint work:

  • Irregular bite
  • Bad prosthetic work
  • Badly placed fillings
  • Bad habits (clenching the jaw, teeth grinding, nail and lip biting)
  • udarac u čeljust ili zglob
  • Etc.

Symptoms that accompany this type of disorder:

  • Cracking (popping) of joints
  • Ringing in the ears (tinnitus)
  • Pain in face and head area
  • Impossibility of opening your mouth wide
  • Irregular jaw opening (jaw moves left and right)
  • Painful jaw opening
  • Etc.

The dysfunction of jaw joint causes imbalance in the whole body and can also cause a whole series of other difficulties. In case you already have got problems with jaw joints, the same examinations and analysis will be used to approach and treat your problem and symptoms. The importance of this branch has been recognized in the area of dental medicine. For more than ten years, IDC Ružević has been monitoring all positive trends in the field of functional diagnostics in EU and the whole world and it has been trying to incorporate them in its work. We are able to solve your problem, if not permanently, then at least help you with solving it as much as we can, using the most modern devices for functional diagnostics and therapy. With the top functional and aesthetic prosthetics, orthodontics, surgery with implantology, the employees of IDC Ružević, with all their knowledge, will reduce your pain and improve your smile. In collaboration with Policlinic Tranfić we solve this problem that is usually complicated for a patient and for most of the dental offices insoluble. Nowadays, when the stress has become a part of our everyday lives, these problems and functional diagnostics are becoming more and more relevant.
